    Allen,
    Great article and very pertinent. To enlarge on the “catheter last” concept, judicious use of grafts is of course not the only way to achieve this. As you know well, other ways to achieve this include TIMELY placement of a fistula (can be achieved by engaging referring physicians in educational activities that encourages TIMELY referrals) and also acute PD which is a great way to avoid CVC placements.
