    There is ample data that older patients may do more poorly on dialysis than with conservative management. Removing the financial incentive to drive elderly patients to dialysis may actually improve their QOL and mortality in addition to being fiscally prudent. As far as cherry picking; there are robust processes to monitor for this but as with anything if your intention is to game the system you will find a way.

    Big is better works if you understand your business, customers and the environment in which you are operating. Change is inevitable; hang in there!
